Field Placement Information Form for  Faculty Liaisons Form Available
You will receive Instructions from your Faculty Liaison to complete a "Field Placement Information" form. (Most Faculty Liaisons use the form found on Blackboard - some have their own form) Complete the form, and return to your Faculty Liaison as they have instructed. Provide a copy to the Social Work office by dropping it in the drop box outside the Social Work office in Mariposa Hall # 4010.

Learning Agreements & Safety Protocol Verification Form Due
To be completed by the Field Instructor with student present and signed by Field Instructor, Student, and Task Supervisor (if applicable). Original to be turned into the Social Work office.  Student to provide a copy to their Faculty Liaison. Field Instructors, Task Supervisors (if applicable),  and student's should keep a copy for themselves. The Learning Agreement form can be found on Blackboard and on the  FORMS page of the Field Education link on the Social Work website.

Field Evaluations Due – Last Day of Field
Field Evaluation form (new blank form or copy from Fall semester) with ORIGINAL signatures (student and Field Instructor) for Fall must be turned into your Faculty Liaison (usually your Practice Instructor).  DO NOT TURN IN TO SOCIAL WORK OFFICE OR Drop Box.  Failure to submit a completed Field Evaluation form to your Faculty Liaison by this date may result in receiving an “Incomplete” in your Field Instruction class (SWRK 195A or SWRK 295A or SWRK 295C for the Fall semester)
