Agencies & Field Instructors

If your agency is interested in providing field placements for our social work students, please do the following:

• Read the Fact Sheet for Agencies Interested In Providing Field Placements for Sacramento State Social Work Students (see Forms below).
• After reading the fact sheet, if your agency qualifies as a field placement site, please complete the Agency Application and Field Instructor Information Form (see Forms below) and mail to the field office.  You may also submit electronically as an attachment to sommer.hayes@csus.edu.
• Upon receipt of these forms, we will notify our Procurement and Contracts Office to send a Social Work Field Placement Agreement directly to your agency in order to establish your agency as an approved facility/program for Social Work Field Instruction.

Field Instructor Trainings

 Advanced registration is required. Field instructors are required to attend a 7 hour "Introduction to Field Instruction" training and "Advanced Field Instruction" every three years thereafter. Please email the field office at sommer.hayes@csus.edu if you wish to register for one of the trainings listed below. In the email, please include the following information:

• Your name & title (indicate MSW or LCSW) (MFTI or other for Task Supervisor)
• The name of the agency in which you will be providing field instruction
• The physical address where you will be providing field instruction
• Your phone number
• The date of the training you would like to attend

A certificate of completion will be emailed after attending. 6 CEUs are offered for the Intro training and 3 CEUs for the Refresher training. Participation in the entire day is required for field instruction certification and/or CEUs. Field instructors must have a current social work student assigned to their agency in order to attend.
