To ensure you receive the requested keys, please bring your completed Key Issue Request form and your OneCard.

You must fill out a new Key Issue Request form with the building, room number, and all authorized signatures. Bring the completed Key Issue Request form and your OneCard to Facilities Management. Your department will be charged for the replacement key.

Bring in your keys to Facilities Management to have them examined by the customer service representatives.

A Velocity Credential, or FOB, is an electronic door key that operates specific doors on campus and can be programmed immediately for the customer and works immediately on the door.  The majority of campus building entrances are now operated by Velocity Credentials.

You must fill out a new Key Issue Request form with the building, room number, and all authorized signatures. Bring completed Key Issue Request form and One Card to Facilities Management and the Customer Service staff will re-activate your credential.

To place a work order, fill out an electronic Work Order Request Form. Alternatively, you can request a Login to our Maintenance Management System. With this login you will be able to request work and then track that request. To request a login please email Facilities Customer Service at fm-work1@csus.edu.

We prioritize each work request according to urgency, using guidelines such as life and safety, the impact to campus, and building system failures. Non-urgent work is prioritized at the shop level on a first-come, first-served basis. Some shops have larger backlogs than others. The shop lead or supervisor can let you know when work should start and finish.

Any problem that poses a life or safety issue, broken glass, loose or fallen ceiling tiles, all plumbing leaks, locked classrooms, elevator problems, heating and cooling issues, spills, and electrical problems.

Tables and chairs should be requested at least 1 month in advance. We have a limited supply of tables and chairs for the campus and they are in high demand.

For consideration of summer work, please submit a work order request with departmental approval and funding in place by January 31st for major projects, and March 31st for minor projects.
