SUBJECT: Request for Summer Address; Mark your Calendars for August 24

Around the second week of August, Senate Office staff will be sending out information on the Senate's Fall Retreat to be held Wednesday, August 24.  For that mailing, unless you tell us otherwise, we will send all information electronically to your CSUS e-mail address or, if you require hard copies, to your CAMPUS mailbox (obviously, we'd like to save on postage expenses).  However, if you will be unable to check your campus mailbox during August, please indicate that in the space below.

Also, if there were a need to convene the 2005-06 Faculty Senate during the summer months, we would need to know how to get in touch with you as quickly as possible.  For that reason, please indicate your home address, telephone number and email address below.  You can either submit this form on-line or mail a hard copy of it before you leave campus for the summer.
