Student Service Learning Orientation

Students participating in service learning projects must complete mandatory service learning orientation (which is separate from, and takes place prior to, the placement site in-person orientation) prior to beginning service hours. As of Fall 2010, Community Engagement Center will provide orientations online through SacCt in lieu of classroom presentations. Orientations will last approximately 20 minutes; students must have access to a computer, web connection, and printer in order to complete the orientation and obtain required service learning forms. Faculty who have submitted a faculty course profile will be contacted soon with information pertaining to the new orientation process.



During the online orientation, students will be required to print out the emergency contact form and service learning time log. Students over the age of 18 will electronically submit the required student guidelines and Informed Consent forms; students who are currently under the age of 18 will need to print out and submit to Community Engagement Center a Minor release completed by a legal parent or guardian prior to starting service. Fingerprinting and TB Testing

Community partners sometimes require fingerprinting and/or TB testing. For information about these two requirements please visit our website

  • Student Guidelines and Limitations

  • Emergency Contact and Information Form – The emergency form is completed and submitted by the student directly to the service placement site.  Students are responsible for notifying the placement site of any medical information or emergency contact changes.

  • Time Log – This form is used by students, the community partner, faculty, and Community Engagement Center to assist in tracking number of hours served by students at the placement site. 

  • Informed Consent, Agreement, Release, Waiver of Liability, and Assumption of Risk

  • Parental Informed Consent, Agreement, Release, Waiver of Liability, and Assumption of Risk for Minors – This form is for students who will not be at least 18 years of age prior to starting the service learning activity. For students who are under the age of 18, their parent or legal guardian must sign and complete this form, and submit to Community Engagement Center prior to the student engaging in service learning at the placement site.
