Students participating in service learning projects must attend a mandatory on-campus service learning orientation prior to begriming service hours. As of Fall 2009, Community Engagement Center will provide group orientations for all students in lieu of classroom presentations. Sessions will start promptly at the times indicated below and will last approximately 20 minutes. Please be on time; late arrivals cannot be accommodated and will need to wait until the start of the next session. Seating availability is on a first-come basis and session will be closed when full. All service learning orientations will be held in the University Union, Hinde Auditorium (first floor).

Forms:
Community Engagement Center will provide students with a copy of each of the forms listed below at the on-campus Student Service Learning Orientation (separate from the placement site orientation) and will collect the student “guidelines” and “consent” forms on-site. Students are encouraged to review the online forms in advance as Community Engagement Center staff will address questions during the presentation.
Student Guidelines and Limitations (.DOC) (.PDF) – Students must sign complete this form and submit to Community Engagement Center prior to engaging in service learning at the placement site.

Emergency Contact and Information Form (.DOC) (.PDF) – The emergency form is completed and submitted by the student directly to the service placement site. Students are responsible for notifying the placement site of any medical information or emergency contact changes.

Time Log (.DOC) (.PDF) – This form is used by students, the community partner, faculty, and Community Engagement Center to assist in tracking number of hours served by students at the placement site.

Informed Consent, Agreement, Release, Waiver of Liability, and Assumption of Risk (.DOC) (.PDF) - This form is for students who are currently at least 18 years of age. These students must sign and complete this form, and submit to Community Engagement Center prior to engaging in service learning at the placement site.
Parental Informed Consent, Agreement, Release, Waiver of Liability, and Assumption of Risk for Minors (.DOC) (.PDF) – This form is for students who will not be at least 18 years of age prior to starting the service learning activity. For students who are under the age of 18, their parent or legal guardian must sign and complete this form, and submit to Community Engagement Center prior to the student engaging in service learning at the placement site.
