Department Signoff Sheets for Culminating Experiences (English 500)

Students must file these within the first two weeks of the semester. Thesis prospecti must be submitted along with the form. Failure to submit will result is in disenrollment from English 500. Be sure to fill out in duplicate and keep one copy for your records. Be sure to attach a photocopy of your signed Advancemnt to Candidacy form with each signoff sheet.
