LOAN DOCUMENTS

Federal Direct Parent (PLUS) Loan

Submit this form if the Parent of a Dependent student wants to apply for a PLUS loan.  Submit your request by the deadline listed below for processing.

Federal Direct GRADUATE/PROFESSIONAL (PLUS) Loan Request Form

Graduate (Masters) / Professional (Doctoral) students must submit this form, if applying for a Graduate/Professional Direct Plus loan in addition to the 2013-2014 Direct Loans.

  • Attending Academic Year / Spring: April 25, 2014

Submit this form if you're requesting a new, reinstatement or change to a Direct Subsidized or Unsubsidized Loan.  Submit your request by the deadline listed below for processing.

VERIFICATION DOCUMENTS

Agency Verification of Food Stamp Benefits

Students maybe required to verify whether someone (including the student) listed in their  household has received food stamp benefits. The information provided will only be used to determine financial aid eligibility and will be held confidential.

Asset Information Form

The Asset Information Form maybe requested from the student/spouse and parent (dependent student), if the asset information on the 2013-2014 was left blank or required during the verification process.

Parent Income Verification

Submit this form if required to verify parent's 2012 income and wage information.

Parent Means of Support

Submit this form if required to verify how the parent(s) supported their household with little or no income in 2012.

Student / Spouse Income Verification

Submit this form to verify the student/spouse 2012 income and wage information.

Student Means of Support

The student must complete the Student Means of Support to verify how they supported their household with little or no income in 2012

Verification of Child Support Paid

Verification maybe required if the student/spouse or parents (if dependent student) reported a value for child support paid on the FAFSA.

Verification of Household Size/Number in College

Verification of household size and number in college maybe required.  Dependent students must provide parents household information.


SATISFACTORY ACADEMIC PROGRESS (SAP)

All SAP Appeals are assigned to your To Do list located your My Sac State Student Center.  You must submit your appeal online AND print a hardcopy to submit to the Financial Aid Office with your documentation by the required deadline

  • Fall 2013 Appeal: Monday, September 30, 2013
  • Spring 2014 Appeal: Friday, February 21, 2014

CCE students in an eligible financial aid program, must complete the CCE Information Request form to determine eligibility for their CCE program.

General Statement Form

Submit this form if requesting/notifying our office in writing of file processing changes.

Marital Status Verification

Submit this form if there is a discrepancy in the marital status reported on the FAFSA for the student and /or parents.  Dependent students must provide both student and parent information.

Student Aid Eligibility Worksheet for Question 23 on FAFSA form

Students who did not answer question 23 on their FAFSA or reported a response indicating that they are ineligible for federal student aid, can use the student aid eligibility worksheet to determine eligibility.  To make corrections go to www.fafsa.gov.

Unclassified Graduates Questionnaire form

(Post-baccalaureate) unclassified graduates maybe required to complete this form for the 2013-2014 academic year.
