Parent Asset Information Verification maybe required from your parent if the asset information on the 2017-2018 financial aid application.
Parent Household Size/Number in College
Verification maybe required to verify the household size and number in college in your parent(s) household.
Parent Income Verification
Verification maybe required to verify your parent(s) 2015 income and wage information.
Parent Marital Status Verification maybe required if there is a discrepancy in the marital status reported on the 2017-2018 financial aid application for your parent(s).
Parent Means of Support
Verification maybe required  to verify how your parent(s) supported their household with little or no income in 2015.

IRS Form 4506-T Use the IRS Form 4506-T to order Tax Transcripts, Verification of Nonfiling, Wage Statements, Account Transcripts and Record Account
Parent Tax Transcript Cover Sheet Attach this cover sheet to your parent(s) 2015 Tax Return Transcript.
Parent Amended Tax (1040x) Transcript Cover Sheet
Attach this cover sheet to either a 2015 Record of Account Transcript for parent(s) or a signed copy of your parent(s) 2015 IRS form 1040X.
Parent Verification of Non-Filing Cover Sheet Your parent indicated on their 2017-2018 Parent Income Verification form that they did not file or were not required to file a 2015 federal tax return.  To verify this status attach a copy of the “Verification of Non-Filing Letter” for each parent in the household who did not file to this cover sheet.

Change in Aid Form

E-Form - Complete this form if you are requesting a change to your 2017-2018 financial aid awards.

Continuing Students: Awarding will begin mid-June.  The Financial Aid & Scholarships Office must determine that you have met the annual Satisfactory Academic Progress review after Spring grades post

College of Continuing Education (CCE) Information Request E-Form Submit this form if you are a CCE student and you have applied for financial aid through one of our CCE programs or there is a change to your enrollment.
Statement to Clarify Verification Items
Based on the review of your financial aid verification documentation, clarification is needed of reported information.  You can access this document, if initiated on your Student Center To Do List.

Unaccompanied Homeless Youth Submit this form if you are required to verify that as of July 1, 2016 you were unaccompanied homeless youth or were risk of being homeless.
Unclassified Graduate Questionnaire Form Post-baccalaureate unclassified graduates maybe required to complete this form for the 2017-2018 academic year.

Budget Increase Appeal

Submit this form if you have documented allowable expenses and you have not maximized your annual aid limits.

Dependency Override Appeal E-Form: The online appeal is available to students on their Student Center To Do List if they submitted their financial aid application with no parent information. 
Consortium Agreement

The Consortium Agreement is for students who are concurrently enrolled at another school.

Parent Loss of Income

Parents of depdent students who have experienced a loss of income for the 2016 or 2017 tax year can submit a loss of income appeal for review.

Student Loss of Income

Students and/or spouse (if married) who have experienced a loss of income for the 2016 or 2017 tax year can submit a loss of income appeal for review. Students must log in using their Saclink account.

If you are disqualified for 2017-2018, SAP Appeals and requested documentation will be assigned to your Student Center To Do List starting mid-June.

Fall 2017 SAP Appeal

Students who do not pass SAP Standards at the end of the 2016-2017 academic year and are enrolling Fall 2017, must be file a Fall SAP Appeal to be considered for Fall aid.  If eligible, Fall SAP Appeals will be assigned to your Student Center To Do List.

September 8, 2017 

Spring 2018 SAP Appeal

Students who do not pass SAP Standards at the end of the 2016-2017 academic year and are enrolling Spring 2018, must file a Spring Appeal to be considered for Spring aid.  If eligible, Spring SAP Appeals will be assigned to your Student Center To Do List.

February 9, 2018 

SAP Appeal Documentation

Students must submit a SAP documentation to complete their appeal.  Refer to the initiated SAP documentation on your Student Center To Do List

Based on SAP Appeal deadline

Official Academic Plan

Students who file an Over Unit SAP Appeal are required to submit an official plan signed by their major / academic advisor

Cumulative Academic Plan

Students who are disqualified for not meeting the Cumulative SAP Standard only are required to submit a Cumulative Academic Plan signed by their major / academic academic advisor
