STEP 2:
Complete Necessary Documents

1. Guardian Scholars Application Form

2. Guardian Scholars Consent Form
   GS Consent.pdf
3. Acceptance of the terms as outlined in the
   Guardian Scholars Student Agreement
   GS Student Agreement.pdf
4. A 1-2 page response to the following essay questions:
   • How has being a former foster youth influenced your desire to pursue higher education?
   • What is your motivation?
   • What are your academic and career goals?
5. Copy of Ward of the Court/Dependant Documentation
6. 2 Letters of Recommendation from a teacher, counselor, social worker, coach, or other
7. A recent photo of yourself

STEP 3:
Submit Your Application

You may hand-deliver your completed application and support materials directly to the Guardian Scholars Office located in Lassen Hall 3014,
or you may mail your completed application to:

STEP 4:
Meeting with the Program Director

Should you meet the requirements, your application will also include a brief interview with the Program Director.

Applications and supporting material
should be submitted
as a complete package to be
eligible for priority consideration.
