SUPPLY ROOM HOURS OF OPERATION:

Monday/Wednesday       9:00am – 12:30pm

                                        2:00pm – 7:00pm

Tuesday/Thursday          3:00pm – 7:00pm

Friday                              8:00am – 3:00pm

Fridays are the best days to come and browse through the supply room materials.  Browsing opportunities may be limited during the week due to the therapy schedule.  If you would like to browse earlier in the week, it is up to supply room worker discretion.  You must ask to enter and receive permission to enter.  The priority is to fill orders.  Please plan to become familiar with materials accordingly.

HOW TO PLACE AN ORDER WITH THE SUPPLY ROOM

  • Please take note of all specific instructions outlined in this form below
  • Orders MUST BE placed 2 hours prior to pick-up time.
  • All orders must be picked up in the Therapy Prep room (2208) from the Supply Room pass through window.
  • Please return items in the same manner in which you received the items. If it came in a bag, return it in its bag.
  • If you discover an item is damaged or broken, please notify a supply room worker immediately.
  • Use exact tracking #’s as stated in the Supply Room inventory database.
  • Be sure to review your order before submitting.
  • The Supply Room is NOT responsible for errors on submission forms.
  • The supply room is only accessible during supply room operation hours. Do not request someone to let you into the supply room when it is not officially open. 
  • If you need to cancel an order because your therapy session has been canceled, please notify the Supply Room staff as soon as possible by calling (916) 278-6603.

Supply Room Order Request Form

If you do not submit an order officially through this online supply room order request form, you will not have supplies for therapy.  Ordering supplies is your responsibility.  Do not attempt to obtain materials last minute outside of clinic policies and procedures. Clinicians are not allowed to enter the supply room to request an order verbally before a therapy session.  Please follow procedures outlined in this form.

Orders that you submit and pickup are due back to the supply room two hours after the designated pick up time, regardless of when you actually picked up your order.

If you have submitted an order and then discover that you need to make a correction to that submitted order, you must indicate in the comments box that the previous order is to be deleted, and that this is a “correction order.”  Please state that specifically.  You then need to make sure that you have re-ordered ALL needed materials, not just the changes or additions that you want.  NOTE: If you submit multiple “correction orders” you run the risk of not having any therapy supplies because you are putting stress on the process of filling orders accurately.   

When picking up orders, you are allowed to pick up supplies 30 minutes prior to the requested pick up times, and no sooner.  If your order has not been picked up within 30 minutes after your requested pick up time, all items will be placed back on the shelves and your priority is then lost for those items for that hour. 

When returning orders, if the supply room is closed, please place your bags neatly on the floor in the Therapy Prep room (2208) underneath the pass through counter to the Supply Room.  Do not place any orders in the hall way, at the front desk or on the work table in the Therapy Prep room.

For weekend checkouts, please indicate pick-up is for Friday and specify pick up time as either 1pm or 2pm. Overnight weekend orders are not to be requested for pick up before 1pm on Fridays

Overnight requests during the week may be picked up between 6:30pm and 7:00pm. Overnight requests for the weekend may be picked up between 1pm and 2:30 pm on Fridays.

Overnight orders during the week and for the weekend are due back to the supply room by 9am the next day, or 9am Monday mornings for weekend checkouts.  Do not expect to be able to keep your overnight check out supplies for your therapy the next day.  They must be returned and redistributed by the supply room staff.

Pick up Time(s) that you need your order

DO NOT SELECT MORE THAN ONE PICK UP HOUR PER ORDER. 

You are allowed to keep your supplies for two hours maximum before they are due back for return.  If you plan on using the same materials during therapy the same day after this two hour window, you must submit a new order for these same items requesting a different pick up time and return the items until ready for pick up per order submission priority. 

NOTE:  You must submit orders for therapy to be picked up just prior to that therapy session.  It is our only way of identifying who has priority.  We assume that if you have ordered items for a particular hour it is because you need those items for therapy at that hour.  Do not request a pick up time that does not coincide with your therapy session.  This is called being professionally courteous and is an expectation of being a graduate clinician in this program.  

 IT IS AGAINST POLICY TO SHARE MATERIALS THAT YOU HAVE CHECKED OUT WITH OTHER INDIVIDUALS.  ALL MATERIALS NEED TO BE RETURNED TO THE SUPPLY ROOM FIRST, AND SUPPLY ROOM WORKERS WILL REDISTRIBUTE MATERIALS PER SUBMITTED ORDERS BY PRIORITY.
