Writing Placement for Graduates (WPG) waiver form (pdf)
Monthly timesheet for graders who are Instructional Student (ISA) assistants. If you are a grader who is a GA you do not need this form. If you are not sure whether you are an ISA or GA, please refer to your appointment letter from the history department.
Advancement to Candidacy form (pdf)
HIST 500 petition ("Special Problems Petition for Culminating Requirement"). Required to register for History 500 because students cannot enroll in HIST 500 through MySacState.
