Sample License Forms

The forms below are sample copies of the agreements and information forms that will be filled out and submitted during the on-line licensing process for 2009-2010 housing. To be ready for the new on-line process, beginning April 6th for returning residents and April 7th for new residents, please have your SacLink Username and your Sac State ID on hand.

If your need to get your SacLink Username go to the SacLink website and follow the instructions to register. To find your Sac State ID (SacLink Username required) login into My Sac State and you will find it at the Sac State ID link on the main page.
