Full and/or part-time employees are required to complete a check-out form upon separation. Please complete the appropriate clearance form and submit to the Payroll Office. Clearance Forms are available in the Payroll Office (Del Norte Hall 3006) or on the Human Resources Forms page under the Payroll section.
