 Before Enrolling

  1. Review employee benefits eligibility criteria and determine which of your family members are eligible for enrollment. You have 60 days from the date of your appointment to request enrollment.
  1. No Dual CalPERS Coverage - Dual CalPERS coverage occurs when you are enrolled in a CalPERS health plan as both a member and a dependent or as a dependent on two enrollments. This duplication of coverage is against the law. When dual CalPERS coverage is discovered, the enrollment that caused the dual coverage will be retroactively canceled. You may be responsible for all costs incurred from the date the dual coverage began.
  1. Review your benefits summary to obtain an overview of the benefits available to you based on your bargaining unit.
  1. Please review all plan information in the benefits packet prior to completing your enrollment forms. An electronic version of the benefits packet is available below. A Benefits Orientation Powerpoint presentation is also available if you were not able to attend a Benefits Orientation or need a refresher.

The Employee Benefits Packet contains general information on the various programs within the CSU benefits package and forms required for enrollment. The packet contains 4 pockets:

  • Double-click a folder (pocket) to open/review contents
  • Double-click a document within a folder (pocket) to select
  • After selecting a document:
    • Click "Open File" in the top right-hand corner of the screen to open the document for viewing
    • Click the "Extract File" button (appears when your mouse is hovering over the document) to save a copy of the file.
  1. Review the Sac State medicaldentalflexcashvisionhealth care reimbursement accountdependent care reimbursement account plan web pages to obtain information regarding plan offerings, search for participating providers, and plan contact information.
  1. Review the rate chart to obtain monthly health premium deduction amounts. Currently dental and vision premiums are fully coverd by the CSU. Dental and vision premiums are included on the rate chart for informational purpose only.

 The Enrollment Process

  1. Complete the  Health/Dental/Vision New Hire Worksheet and the CalPERS Declaration of Health Coverage forms, and submit to:

  1. Complete  a Dependent Care/Health Care Reimbursement Account Plans Enrollment Authorization form to enroll in the HCRA and/or DCRA plan. (optional)
  1. Provide a copy of supporting documentation required for enrollment of family members. See Benefits Eligibility page for required documentation.
  1. Coverage effective dates are based on the date your enrollment forms are submitted to the Benefits Office.
  1. Review Voluntary Benefits offerings and submit enrollment forms as necessary.
  1. Upon submitting enrollment forms to the Benefits Office - your enrollment will be keyed into the system. You will be contacted within a few days to return to the Benefits Office to review and sign the official enrollment forms.

 After Enrolling

  1. Review your pay warrant:
    • After enrolling you should carefully check your pay warrant stub to verify that the correct premiums are being deducted from your pay. Premium information can be found on the Benefit Rate Chart.
  1. Do You Need to Use Services Immediately?
    • Your health benefit enrollment forms confirm the effective date for your coverage. However, it may take up to 60 days after you enroll before the insurance companies show a record of your enrollment. Be sure to keep a copy of your official enrollment forms mailed out to your attention after the Benefits Officer has signed.  Verify benefit premium deductions on your pay warrant stub to ensure your enrollment has processed completely.  The presence of a premium deduction on your pay warrant stub will confirm that premium payment has been made to the insurance carrier.
    • If you know at the time of enrollment that you will need to use services immediately after your new coverage takes effect, please inform the Benefits Office at the time of enrollment. If you need to use the services prior to your insurance carrier having a record of your enrollment, please contact the Office of Human Resources – Benefits at 916-278-6213.
