Life Events - What You Need to Know

In general, you have 60 days from the date of an eligible life event to notify the Office of Human Resources - Benefits in order to prevent delays and/or penalties. Listed below are some of the life events which will require you to contact the Office of Human Resources.

Changing Your Name

Moving

Getting Married

Registering a Domestic Partnership

Birth or Adoption of a Child

Getting Divorced

Terminating a Domestic Partnership

Loss/Gain of Alternate Health Coverage

Economically Dependent Children

Death of a Covered Dependent

Changing Your Name

  • You will need to notify the Payroll office in order to change your name. The paperwork you complete in Payroll will update all necessary systems and insurance providers.

Moving

  • You have two options to notify the campus of an address change:
    • Complete an Employee Action Request form in the Payroll Office.
    • Sign-in to your MySacState account to update your address online.
  • Since you must choose a CalPERS health plan that provides coverage in your work or home ZIP Code, a change in your address could mean you have to change health plans. Contact the Benefits Office as soon as possible to provide us your new address and to determine if a health plan change is needed. You can use the CalPERS Health Plan Search By ZIP Code online service to see which plans are available in your new ZIP Code.

Getting Married

  • Contact the Benefits Office to add your new spouse (and eligible stepchildren) to your health coverage. We will need a copy of your marriage certificate.
  • Coverage will be effective the first of the month following the date you submit your enrollment request to the Benefits Office.

Registering a Domestic Partnership

  • Contact the Benefits Office to add your new domestic partner (and eligible stepchildren) to your health coverage. We will need a copy of the declaration of domestic partnership.
  • Coverage will be effective the first of the month following the date you submit your enrollment request to the Benefits Office.

Birth or Adoption of a Child

  • Contact the Benefits Office as soon as possible to add your new child to your health coverage. We will need a copy of the adoption papers.
  • A copy of the birth certificate or adoption decree is required.

Getting Divorced

  • Contact the Benefits Office to have your ex-spouse removed from your health plan.
  • It is mandatory that the coverage will terminate the first of the month following the date of the divorce.
  • A copy of the divorce decree is required.

Terminating a Domestic Partnership

  • Contact the Benefits Office to have your former domestic partner removed from your health plan.
  • A copy of the Termination of Domestic Partnership is required.

Loss/Gain of Alternate Health Coverage

  • If you initially declined (or cancelled) enrollment for yourself or your dependents (including your spouse) because you had other private or other health coverage at that time, you may be able to enroll in CSU health and dental plan if the other coverage involuntarily ends. To qualify, you will need to request enrollment within 60 days after the other coverage ends and provide proof that the other coverage has ended.
  • If you initially enrolled in CSU health and/or dental and you gain alternate non-CSU coverage, you can enroll into the FlexCash program by completing new enrollment forms within 60 days of the status change event

Economically Dependent Children

  • Another person's child under age 26 may be eligible for coverage if a parent-child relationship exists. 

Death of a Covered Dependent

  • Please report the death of a covered dependent to the Office of Human Resources - benefits as soon as possible so that the corresponding transaction may be processed in a timely manner.
