Health Plans

  • Monthly plan premium costs shared by the CSU and the employee. 
  • Employee contribution rates vary depending on the plan selected and the number of dependents enrolled.
  • Before enrolling, review the Benefits Rate Chart and the information provided below on each plan.
  • Use the Health Plan Chooser online tool to help make a decision regarding what plan is best for you.
  • Medical insurance is administered by CalPERS.

Health Maintenance Organizations (HMOs)

Employees select a primary care physician (PCP) who coordinates all care including referrals to specialists.  You are responsible for co-payments for some services; you have no deductibles and no claim forms to complete.  HMOs are available only in designated California service areas.  Use the CalPERS Health Plan Search By ZIP Code online service to see which plans are available in your area.

Anthem Blue Cross Traditional HMO
Individual practice plan where the employee must use contracted doctors/hospitals designated on plan lists for all services.

Anthem Blue Cross Select HMO
Individual practice plan where the employee must use contracted doctors/hospitals designated on plan lists for all services.

  • Same level of benefits as Anthem Blue Cross Traditional HMO, but with a lower monthly premium cost and a smaller network of medical groups.

Blue Shield Access+ HMO 
Individual practice plan where the employee must use contracted doctors/hospitals designated on plan lists for all services.

  • Generally, no deductibles and no claim forms are required.

Blue Shield NetValue 
Individual practice plan where the employee must use contracted doctors/hospitals designated on plan lists for all services.

  • Same level of benefits as Blue Shield Access+ HMO, but with a lower monthly premium cost and a smaller network of medical groups.
  • For more information refer to the Evidence of Coverage.

Kaiser
Clinic setting HMO plan where the employee must use Kaiser facilities within the plan area for all services.

United HealthCare
Individual practice plan where the employee must use contracted doctors/hospitals designated on plan lists for all services.

Preferred Provider Organization (PPO)

Employees can select to have services with any provider of their choice, but you get maximum benefit coverage when you use the PPO network of physicians and hospitals.


PERS Choice and PERS Care (administered by Anthem Blue Cross)
  • Members may select primary care provider and specialists without referral.
  • Non-network providers may be used, but co-payments will be higher.
  • Annual deductibles must be met before some benefits apply.

PERS Select (administered by Anthem Blue Cross)

  • Same level of benefits as PERS Choice at a lower monthly premium cost.
  • Available in 54 of the 58 California counties (not available in Alameda, Marin, Placer, or Solano counties, or out-of-state).
