Dependent Fee Waiver Procedures

  1. All interested employees must first complete a Certificate of Eligibility form and submit it to our office. Once you have been determined eligible, you will be contacted via email and a copy of the form will be mailed to you. If the student is already admitted to a University, additional forms will be forwarded to you.

  2. If the student is not yet admitted, they will need to apply to a University using www.csumentor.edu. The additional forms will be forwarded to you once you have notified our office that the student has been admitted.


  3. Submit a Fee Waiver - Transfer of Benefit form to our office with all appropriate signatures. The form can be obtained from the Professional Development/Training Office in Sacramento Hall, Room 173. Please do not remove any copies of the form; they will be mailed to you upon completion of processing your paperwork. In addition, the student will need to submit a Degree Program Plan outlining their next four semesters of coursework.

Note: If applying the fee waiver to your Domestic Partner, you must provide a copy of your "Declaration of Domestic Partnership" form from the California Secretary of State, as proof of domestic registered partnership.
