Vital records are a state responsibility. In the past, the U.S. Dept. of Health & Human Services provided a publiction , Where To Write for Vital Records, which provided addresses for each state. However the federal government no longer publishs it, and the private sector has taken over this area.
The "Vital Records Directory " world web site lists where to write to obtain vital records for each state.
