Requesting a Letter of Recommendation

If you need more copies of a letter previously written, start at step 2. If you are requesting a new/first time letter, then email me (at left). After I agree to write the letter, please e-mail to me:
- Your completed application, if possible. If not, then:
- Your personal statement
- Unofficial transcripts from CSUS
- Your full name as it appears on your application
- AAMC, or AADSAS numbers, if applicable
- What semester and year did you take my course(s)?
- Which course(s) did you take with me?
- E-mail address that you check regularly
- Average hours employed each week and employer
- Name of addressee (if other than “Selection Committee”)
- Any other relevant material
- If your recommendation will not be sent through the internet, then mail, deliver (or attach to my cork board):
- Any forms that you need sent with your letter. The forms should be filled out as completely as possible by you (including my name and contact information, included below).
- Place the completed forms into stamped, unsealed, pre-addressed, envelopes (either your own, or, CSUS envelopes). CSUS envelopes are available at the department office or from me. If you no longer come to campus, you may send postage and pre-addressed self-adhesive labels instead of envelopes.
- E-mail a reminder to me one week before you want your letter to leave my office
