PCI Self Identify Form

Please fill out the following form if you process any credit cards on the Sacramento State Network. After you have filled out the form a member of the Information Security Office will contact you about scheduling a time to perform a self assessment on Payment Card Industry compliance. Currently all assessments are in preparation for the Chancellors Office Audit.
