Faculty Evaluation Services

Introduction

Faculty Evaluation services are provided to departments for providing statistical analysis of the evaluation of faculty members.

Requirements

  1. Use only the California State University, Sacramento Faculty Evaluation Questionnaire scan form (NCS Trans-Optic # MP06-77700).
  2. A maximum of 50 questions can be used
  3. Code a header form for each course being evaluated. the course number must be marked in the Course Code section and the YR section must be marked as 55.
  4. Group all course headers together; do not separate courses. You should have one group of course headers and one group of student responses.
  5. Marks must be dark and precise, using a # 2 pencil only. The scanner will reject forms marked in ink.
  6. Check all student responses for errors. The Course Code section must be marked with the course code. The YR section must be marked with the last two digits of the year being evaluated. Incorrectly coded responses will not be processed.
  7. Do not use paper clips or staples. Forms must be straight and clean of any extraneous marks.
  8. All forms must be facing the same direction.
  9. All written comments must be contained within the comments section of the form.
  10. Submit a questionnaire with each evaluation request, even if you have done so in the past.
  11. Complete a Faculty evaluation form (also available at the Help Desk Counter in ARC 2005) and submit it along with the questionnaire, course headers and the student responses to the Help Desk (ARC 2005).

Courses not processed due to invalid course codes or incorrectly coded forms will be retuned to the department for correction.

Special Note:

Courses without standard codes (codes made up by the department or codes not assigned to an instructor in the Class Schedule) must be processed separately from the standard evaluations. For these evaluations to be processed, you must also submit a memo containing the course information. The required information for each course is course code, instructors name, course number, course section and course title.

Turn Around Time

  1. Allow at least 15 business days for the evaluations to be processed.
  2. You will be notified via phone when the evaluations have been processed.
