Please be careful of deadlines. Office of Graduate Studies deadlines in fall 2011 for forms effective spring 2012 have been moved temporarily moved up to accomodate systems upgrades.
*September 9, 2011 was the new deadine. OGS is no longer accepting these forms for spring 2012.

Public Policy and Administration

• Add Permit (student responsible for submitting to proper office).
  To add during first four weeks of class only.
• Add/Drop/Withdraw Petition (student responsible for submitting to proper office).
  To add/drop/withdraw after Census Date.
• Advancement to Candidacy Form*.
• Human Subjects Policy.
• Human Subjects Application.
• Incomplete Grade Petition.
• Internship Proposal Form, PPA 295 Examples of internship papers.
• Internship Waiver Form.
• Program Application and Recommendation Forms (at least three recommendations required) -- application page.
• Special Problems Petition, PPA 299.
• Student Handbook.

Admissions & Records Official Forms Page
Forms Include:

• Add/Drop Petition.
• Repeat Petition.
• Transcript Request Form.
• To change your student record information use the following form: Address, Phone Number, Information Release Authorization
• To change your name go to the Admissions and Records Counter in Lassen Hall first floor lobby with a photo ID.
• Withdrawing from the Semester.
