| 1. | Do not use this form if you wish to test at a CSU campus other than Sacramento. Follow the instructions in the ELM/EPT information bulletin. |
| 2. | If you do not feel comfortable transmitting personal information, mail or bring the completed form to the CSUS Testing Center, Lassen Hall, room 2302. |
| 3. | We must have either the mail-in or on-line form in our office by close of business day of the registration deadline date. |
| 4. | Students with disabilities must request accommodations through Services to Students with Disabilities office, LSN-1008, 278-6955 and the Testing Center. |
