Test Instruction Forms
Test instruction forms are to be completed by faculty and submitted with the exam.
We require one exam for each student. Exams are not copied. Do not email tests.
Faxed tests require prior approval.
The Special Testing Accommodation form for students registered with Services to
Students with Disabilities (SSWD), and authorized special testing accommodations, is

The Make up test form, for students taking missed classroom exams, exams with
class conflicts, or distance education courses, is available here as a PDF file.
The Challenge Exam form for students taking an examination in lieu of the
coursework is available here as a PDF file.
