Creating a "fillable form" using Adobe Acrobat? Be aware that Acrobat 8 Professional ships with Adobe LiveCycle Designer, an application used to create advanced PDF forms. The learning curve for this software is quite steep, so we suggest you only use it if you're creating complicated forms. For most forms, use the Acrobat forms tools, available from the Advanced Editing menu in Acrobat. See the workshop handout, Adobe Acrobat: Introduction, for help getting started with basic PDF forms.

GUEST ACCOUNT REQUEST FORM

SacCT faculty may request a guest account for a guest lecturer or other non-student participant by completing this form. If requesting an account for an individual on campus, please provide the guest's SacLink account name.

Guest accounts will not be created without approval of the SacCT course owner. The guest account name and password will be sent to the course owner within two working days.

All users of SacCT must be placed in a Global Database before they can access a course. Faculty are automatically placed in the database when they request a new course. Registered students are placed in the database when faculty request an SIS+ student list be loaded into SacCT by completing the SacCT Course Status form.
