Declaration of Major Form-For use to change your major or minor to Women's Studies. Submit to the department office (AMD 561B) for Department Chair's signature.

Major Requirement Form-Lists both the required core courses and the elective options for majors.

Minor Requirment Form-Lists both the required core courses and the elective options for minors.

Major Advising Form-For use in choosing your major courses in consultation with your Faculty Advisor. 

Minor Advising Form-For use in choosing your minor courses in consulatation with your Faculty Advisor.

WOMS 199/299 Petition-For use when requesting an Independent Study course.  This form must be filled out in consulation with a Women's Studies Faculty Sponsor and signed by the Department Chair.  All petitions must be processed no later than the 4th week of the semester.

Graduation Application-For use to apply for graduation. Graduation applications should be filed one year priorto expected graduation date.

Add Petition-For use during the first 2 weeks of semester only.

Add/Drop Petition-For use to add or drop courses after the 2nd week of the semeter.
