Faculty and Staff: Reporting an incident of intimate partner violence or sexual assault

Faculty and staff should use the following forms to report an incident of sexual assault or intimate partner violence to the Women’s Resource Center.

If a student reports an incident to you, either formally or informally, please use one of these forms to forward the information on to the Women’s Resource Center. You can also call us to have an advocate meet with the student. The advocate can coordinate services for the student, should he or she request them.
