STATEMENT BELOW BEFORE YOU USE THIS LETTER/FORM!!!
This is a sample
letter for you to use in soliciting participation from your chosen
organization. You may want to e-mail them first or actually send a
snail-mail. Fill in the blanks as you see fit. As you can see from the
letter, you are to collect the “Informed Consent Form” from the
interviewee after you conduct your interview. This form is included in the
letter. This form is used to let our participants know that we are
concerned about protection of their identities and we are not going to use
information they provide to us for purposes other than our class. If you
have any questions about this document, let me know.
