Medical Records

All medical records are strictly confidential (Notice of Privacy)Information from records will not  be released to anyone without written authorization of the patient, except as required by law.  Student Health & Counseling Services Medical Records can assist you with the following services.

  • Requesting medical records for yourself
  • Clearing Immunization Holds
  • Sending medical records from Student Health & Counseling Services to another health facility or provider


Complete medical records are maintained on all Student Health & Counseling Services patients. Information in student medical records is confidential, and access is restricted to authorized SHCS personnel. Except in a life-threatening situation, SHCS may not notify parents, professors or anyone else regarding your health care or unless you are a minor. It is important that you notify your provider should your parent require medical information and if your healthcare provider may provide them information.

Retention of medical records

Electronic records will be retained for a period of 10 years after the last date of service.

Copies of Medical Records

Patients who wish to obtain a copy of their Electronic Records may contact the Medical Records department at 916-278-6461. If you wish to release your medical record to anyone, including yourself, you must first sign a release form.  A paper copy of this form is also available at the Student Health Center.

Records may take up to two weeks for processing. Cost for records release is $15.00.
