CAMP Application
To apply for the CAMP program students must submit the following required documents:
Note: Once you complete each form and submit it, we will receive a copy and contact you.
- Personal Information Form
- Autobiography
- Letter of Recommendation Form (Only one letter required but two recommended)
- Additional Documentation Form
Submission Instructions:
Students can access the 1040 Tax Return Forms and W2 Forms through their parent(s) tax return paperwork. Students can access a copy of their unofficial transcript through their high school counselors. Students are encouraged to ask teachers or counselors for letters of recommendation or to complete the recommendation forms within the CAMP application. Students can also provide CAMP staff with the recommenders’ contact information to contact them on their behalf. A copy of the EFC can be obtained through FAFSA on the Student Aid Report.
