Emergency Grant Non-Qualified Scholarship Category Criteria
You are required to electronically submit at least one documentation for each category of support that you are requesting. Please note, if you are seeking an Eviction Avoidance grant in the housing category, you must provide at least 2 pieces of documentation; your fully executed lease agreement and at least one additional documentation that reflects your rent is past due.
Supporting Documentation to Provide
The information below will provide you with examples of the kind of supporting documentation you can provide, as well as the total dollar amount that you may request for each category. You are allowed to request funds based on your documentation, and not more than the total dollar amount allowed for each category.
*Please note: If your need is related to loss of income, please also submit proof of loss of income (such as a letter from your employer about termination and or/copies of 3 paystubs showing reduced income)
A letter from your childcare provider verifying the hours of care provided each week, and the total amount due
Please note: This category does not include technological devices such as personal computers, laptops, or tablet type devices.
- A document that shows the total costs of the course materials needed.
- If course materials were stolen, provide a copy of law enforcement report verifying the items stolen.
- 3 copies of your most recent paystubs that show your work hours were reduced.
- A letter from your former employer showing that you were terminated/laid off, or that your work hours were reduced.
- Correspondence from a county government office that shows you had a change in benefits under CalFresh or Cash Aid.
Please do not submit your personal health records.
- Copies of recent medical bills that include the date of service and the total amount due.
- A letter from your medical provider indicating the procedure was medically necessary may also be requested.
For Eviction Avoidance: you must provide multiple documents, as below:
- A copy of your signed lease agreement, including the breakdown of each individual resident’s total rental payment due monthly.
- A signed letter from your property manager/landlord, or a 3 day pay or quit notice from your property manager/landlord, that shows your total amount due to avoid eviction. For Move-In Support
- A copy of your signed lease agreement with a confirmed move in date, including the breakdown of each individual resident’s total rental payment due monthly.
- If seeking support to cover security deposits, ensure that the lease agreement includes this information.
Please note: Technology requests can be made when your computer/laptop has been stolen, damaged, or is in need of repair or replacement. Grants cannot be used to replace/repair Air pods/Earphone/Headphone devices gaming devices, and any other devices that are deemed unnecessary to aid in completion of courses.
- If your equipment was stolen, provide a copy of a law enforcement report verifying the items were stolen.
- Photo images of damaged equipment to be replaced.
- If the damaged equipment can be repaired, please provide an estimate of the cost to replace parts and or complete the repair.
Please note: Car insurance is required in the state of California. If you do not have proof of motor vehicle insurance, we cannot assist you with costs to repair your vehicle after an accident. The grant is not allowable to pay car insurance premiums.
Emergency Travel for bereavement or medical emergency
- If the need is related to bereavement after the death of a family member or related to a medical emergency, the student will be asked to produce a written statement from a healthcare professional that indicates there is a need for the student to travel.
- A document/screenshot of the cost of travel. Travel can be by plane, train, or bus. If you are seeking assistance with covering the costs of fuel, a Google Map search can be submitted to show the mileage from your primary residence to the location where you are going to travel.
Car Safety Repair, Including Damaged or Stolen Parts
- Documentation of ownership of the vehicle. If you are using a family/friend’s vehicle, then the owner of the vehicle must write a letter confirming that you have permission to use their vehicle as your primary commuter vehicle.
- If you are seeking assistance with a safety vehicle repair, or assistance with costs after a motor vehicle accident, you will need to submit an estimate of the costs for repair and proof of vehicle insurance. The estimate must come from a professional auto-repair business, or your insurance company.
- Documentation of any rental car and costs.
Fuel Purchase for Commuting
- If you are seeking assistance with costs of fuel, your primary address of residence must be 50 miles or greater one-way travel to campus, and a Google Map search can be submitted as proof.
- A copy of a past due notice for utility costs that indicated that utility service will be suspended to the student’s primary residence, due to lack of payment.
- If you have a roommate, then in the narrative of your application please provide a written description of how much you pay to cover the costs of the home utilities.
