Interview forms

  • Candidate Accommodation Form - All Candidate Accommodations must be preapproved by the department Search Committee. This is for any candidate that will travel to SacState for an interview, tour, or meeting. It is to arrange their travel, even if they are driving their personal vehicles and are local, the Candidate Accommodations Form is to be completed.

College of Arts & Letters Range Elevation business process:

  • The college will send an email with the proposed faculty to the department.
  • The department confirms the names.
  • The college will notify Faculty Affairs per the MOU. See inside Faculty Affairs - Range Elevation for more information.
  • Faculty Affairs sends a final list to the department chairs and the college.
  • The college notifies the department of eligible instructors, a formal memo is to be posted by the dept on an employee board in a public place (as agreed upon/past practice by the department) with a specified timeframe, and an acknowledgment form for faculty to sign.
  • Faculty are notified by the department of their eligibility. Faculty sign the acknowledgement form and return it to the department either accepting or decline the review process.
  • The signed acknowledgement form is sent to the college by the department.
  • College adds, if needed, a faculty WPAF in Teams/OnDrive and notifies the department chair that they can access & review the WPAF.
  • Department chair opens the WPAF (per their department policy) to the faculty to upload any needed documentation and collects verification of closed WPAF.
  • The department chair informs the college of their review recommendation per the MOU. The college informs faculty of the Chair's recommendation letter per the MOU.
  • The college sends out the Dean's final decision to the faculty and sends the Provost & Faculty Affairs the copy of the final decision for range elevation per the MOU.
